Chronic disease services are available for all ages. We will help provide medication refills to remain consistent with your care.

Need a dentist, eyeglasses or lab work?

After an initial General Care visit we will help refer you to a cash-pay specialist like:

  • Dentists
  • Ophthalmologists
  • Physical Therapists

If you require blood work or imaging like x-rays we can make recommendations that can save you money.

1I have a rash

D2C Provider: Okay. Have you been exposed to anything unusual or different lately, like a new soap or detergent? Or even a new medication?
Patient: No, I haven't changed anything in my routine.
D2C Provider: Based on your symptoms, it's possible that you have a skin infection. Can you put the phone’s camera closer to your arm and hold it steady?

D2C Provider: Yes, it looks like you have a bacterial skin infection. I recommend that you apply ABC [a prescription topical ointment] to the affected area and avoid touching or scratching the rash. I see your preferred pharmacy is Joe’s Pharmacy on 45th street, is that correct?
Patient: Yes, that’s right.
D2C Provider: Great – I’ll send it there. If the rash does not improve within a few days, let me know via the app and I will help you make an appointment with a dermatologist for further evaluation and treatment.

2I have abdominal pain
Provider: Hello, this is Dr. Smith. How can I help you today?
Patient: Hi, Dr. Smith. I'm experiencing some abdominal pain and I'm not sure what to do.
Provider: I'm sorry to hear that. Can you tell me more about your symptoms? How long have you been experiencing the pain?
Patient: The pain started about 2 days ago. It's a sharp pain on my right side and it's been getting worse.
Provider: Okay. Have you been running a fever or experiencing any nausea or vomiting?

3I have itchy eyes and a runny nose

Provider: I'm sorry to hear that. Can you tell me how long have you been experiencing these symptoms?
Patient: It's been going on over a week now.

4I'm not sure if I need to go to the ER

Patient: Hi, Nurse Johnson. I’m feeling very sick and I think I need to go to the ER.
Provider: I'm sorry to hear that. Can you tell me more about your symptoms?
Patient: I've been feeling really weak and tired, and I have a high fever and body aches.
Provider: Okay. Have you been experiencing any shortness of breath or chest pain?
Patient: No, just the fever and body aches.
Provider: I see in the app that you have been seen before and I have your medical history. Are you in a private room so that we can do a physical assessment?

Provider: Based on our discussion and the visual assessment, it's likely that you have the flu. I recommend that you rest, drink plenty of fluids, and either take ABC or XYZ [over-the-counter medications] to manage your fever and body aches.
